New Customer Application Forms. (Left-click the link
to view & print the forms in your browser, or right-click the link
and choose "Save Target As..." to save the forms to your
hard drive.)
Please be sure to complete all three forms, and have
an owner sign them to insure expedient processing.
Once completed, fax to our Eagan Distribution Center
at: 651.450.0844.
Upon receipt, processing will begin. Once approved, a Bell Customer Representative
will contact you to insure you have all the tools necessary to begin
a smooth transition into partnering together. We look forward to that
opportunity.
